A pushed to-do is showing outside the Pushed folder with a little send badge — what is that?

That's a to-do you reactivated on your device (dragged out of the device's Synced area). It's a normal to-do again, but the badge marks it as already handed off, so the app shows that badge instead of a push button — it won't create a duplicate in your task app.
